Dear Dr Kurac,

Re: Mr Andy Williams, aged 65

Mr Andy Williams, who is obese with weight of 155.5kg, requires education regarding treatment options for weight loss including surgery.

Mr Williams works as a radiologist and has recently divorced. Depression associated with financial issues was noted. Apart from that, he has a significant medical history of type 2 diabetes mellitus, hypertension and gout. Current medication includes insulin 30 and 70 units before breakfast and after dinner respectively as well as metformin 850mg twice daily, atorvastatin 10 mg lisinopril, nefidipine, allopurinol and over- the- counter vitamin B12 supplement. Body mass index of 46.6kg/m2, morning blood glucose ranging between 100 - 130mg/dl and triglyceride 201mg/dl were noted.

In addition, continuous weight gain every decade since childhood was recorded. Involved in commercial and medical weight - loss program; however, weight was regained due to abstinence. Consultation by the dietician was taken. Eventhough, bariatric surgery was advised, he showed reluctancy due to fear of post - surgical complications.

Based on the aforementioned circumstances, adequate information regarding weight - loss treatment would be profoundly beneficial.

In case additional information is required, please do not hesitate to contact me.

Yours Sincerely,
