Dear Ms Howard,

Re:Ms Betsie Anderson, aged 39

Iam writing to update the current health status of Ms Betsie Anderson, who is recuperating from urinary tract infection, she requires continuing care following discharge today.

On 16 February 2020,Ms Andreson was admitted with complaints of urinary incontinence, low grade feverand sudden weight loss;therefore, she underwent multiple investigations. Acetaminophen 650 and ciprofloxacin was commenced. She was assisted with activities of daily living. Daily assessment of loss of consciousness and blood pressure monitoring were done. Presently, she is oriented and ready to be discharge.

As per discharge plan, Ms Anderson was advised to continue levothyroxine, protonix, colace, ciprofloxacin, multivitamin with calcium and aricept. It would be greatly, appreciated if you could monitor her vital signs daily. Kindly observe him for any signs of dehydration and orthostatic nlood pressure.Urine analysis and labs are needa to repeat with in two weeks and consult with Dr Alex Rose. Please note, she requires walkee, cane and bedside commode.
