Dear Mrs Adison,
Re:Mrs Margarat Adam, aged 76
Mrs Margarat Adam, who is recuperating from the left total hip replacement, requires home visits for continuing care and monitoring. She is being discharged today.
On 21 July 2021 Mrs Adam was admitted followed by a fall at home. An x-ray revealed fracture on the left hip acetabulam;therefore, she underwent, surgery successfully. Post-poeratively, she was disoriented and her pain was managed with paracetamol.She has been commenced on alendronic acid 70mg, calcium and vitamin c along with anticoagulant. Physiotherapy and daily dressing were provided.
Presently, Mrs Adam is able to walk with stick;however, she has slight swelling on hip region. Self administration of deltaparin has started with supervision, which need to be stop on 17 August 2021.
As per discharge plan, it would be greatly appreciated if you could provide home visits every two weeks to monitor her medication compliance, wound and swelling. Kindly, encourage her to continue the exercise regime. A wound dressing has scheduled on 06 September 2021 and she was instructed to start antiembolism stockings. An appointment for medicine revival is due to be on 22 September 2021.
