Dear Ms Forest,


Re:Mr John Alex Frederick, aged 32.


Iam writing on behalf of Ms Noarah, whose husband Mr John Alex Frederick. He is recuperating from a fall, requires home visits to improve his mobility.


Mr Frederick is married and lives with wife.He had a stroke three years ago;subsequently, he walks with limp due to impaired balance. He has an appointment with speech pathologist every two weekly for slurred speech.


On 12 July 2019,Mr Frederick had a fall following an extreme headache. Left knee badly got injured. Since that time, He has been in our care. He was assisted with activities of daily living incuding daily dressings. Presently, his wound is healing well and able to walk with help of wife;however,he has pain while walking.He was advised to walk with stick.


Based on the aforementioned circumstances, it would be greatly appreciated if you could provide daily home visits to continue physiotherapy to improve his mobility.
