Re: Ms Jane Simms,aged 46


I am referring Ms Jane Simms, who has been diagnosed with progressive multiple sclerosis, requires continuing care and thorough management from your service following her discharge tomorrow.


On 26 July 2018, Ms Simms was admitted to the hospital with exacerbating symptoms of multiple sclerosis. During hospitalization, she was treated with prothiaden 150mg, dosulepin hydrochloride along with ACTH and corticosteroids. Dressing of pressure area on the right buttock was performed. Currently, wound has healed well and she is ready to be discharged.


Ms Simms, a invalid pensioner, lives with her sister. She has had multiple sclerosis for the past 20 years. She also has depression which escalated after the retirement. She is obese for which she is on low calorie diet.


Pertaining to the above details, it would be profoundly beneficial if you could monitor Ms Simm's medications, body weight as well as skin integrity. Kindly ensure on going exercises program and encourage new activities and interests.
