Re:Ms Monica Osburn, aged 69

Ms Monica Osburn, who has been diagnosed with hypertension and depression requires continuing care aa sshe is moving to your area.

Ms Osburn lives alone in a rented house, who is an aged pensioner. She has had hypertension and depression for the last 10 years and 2 years respectively, takes antihypertensive and anti- depressant; however,not compliant with medicines. Excessive alcohol intake was reported.

It would be greatly appreciated, if you could provide regular monitoring of medication compliance and intake of alcohol. Intract with family and other social activities were recommended to avoid further deterioration of depression.
