Dear supervisor,

Re:Mr Ted Watson, aged 72

Mr Ted Watson, who has been recuperating from dyamic hip screw of the right femur. Is being reffered for continuing care and assistants for activities of daily living following the discharge today.

10 May 2018,Mr Watson was admitted with decreased mobility for which surgery ws performed successfully. During hospitalisation, an episode of confusion was recoded. Urinary tract infection was managed with ciplox, ural and paracetamol. During the following days, a catherer has been inserted for incontinence. Regular daily dressings were provided on right arterial leg ulcer. Presently, mobilisation is possible withsupport of a pick-up frame.

Mr Watson lives alone but is frequently vidited by his supportive daughter. Allergic to Amoxicillin and penicillin was reported. Help and support was offered by Ivanno day centre and Heiflberg council. Concerned regarding managing at home after discharge was expressed.

Placed on the aforementioned circumstances, regular monitoring and appropriate management would be extrimily beneficial to improve Mr watson Health condition.Urinary catheter should be replaced every 6 weekly.
