RE: Ms Guana Ferrocia, DOB 01/06/1964
I am writing regarding Ms Ferrocia, who was admitted to our hospital for haemorrhoids grade 4 on 29/10/2021. Due to making excellent progress, she is being referred to your surgical unit and now requires your continued care and management.
Ms Ferrocia was presented with rectal bleeding with burning pain, prolapsed and tenderness as evidenced by physical examination. She also complained of constipation and hard stools due to poor dietary habits. She was then prescribed tylenol for pain, lidocaine for itching, and proton pump inhibitors for gastritis. She was treated with intravenous fluids and an enema.
She has signed the consent form, however, she is allergic to aspirin and sensitive to penicillin.
She has been also discussed with the management of post-operative and pre-operative assessment has been explained.
She has been advised to soak the area in warm water and to maintain a diet with high fiber to prevent constipation. She has a medical history of type 2 diabetes mellitus, inflamed bowel syndrome, and passing out fresh blood while defecating. In 2018, she had a colonoscopy and inflammation of the appendix in January 2020.
It would be greatly appreciated if you could provide further care and support for Ms Ferrocia.
