Re:Mrs Caderina Piper,aged 90
I am referring Mrs Caderina Piper,who has been diagnosed with post traumatic stress disorder, requires continuing care and thorough management at your service following her discharge today.
Mrs Piper initially presented in October 2016 with the complainants of delusion and confusion owing to a robbery attempt in her house. She was discharged to a senior living building after two months. In March 2017, she re- presented with worsening symptoms and subsequently moved to another facility. In November 2017,she reported hallucination along with severe paranoia therefore, she was hospitalized. One week following her discharge to a senior apartment, her symptoms recurred rapidly.
Mrs Piper, a widow, lives independently, has no relatives except a nephew who talks to her monthly. She has had osteoarthritis, urinary incontinence, and pernicious anemia. She is on Haloperidol 0.025-1.0mg daily.
Pertaining to the aforementioned details, it would be appreciated if you could provide on going cognitive behavioral and exposure therapy. Kindly identify ineffective coping mechanisms and ensure adequate time to discuss and explore her feelings. It is extremely beneficial to develop appropriate support systems as well as referrals to future therapies. Please monitor her medications whereas, encourage her in accepting emotions. Kindly call
