Dear Sir/Madam,
Re: Mr Ted Watson, aged 72
I am wtiting to refer Mr Ted Watson, who has been recovering from Dynamic hip screw of right neck of femur. He requires continuing care and asistance in daily living activities. He is being discharged today.
Mr Watson was admitted to hospital on 10 May 2018 with decreased mobility and he underwent surgery successfully. During hospitalisation, he had an episide of confusion. He was febrile due to urinary tract infection which was treated with ciplox, ural and paracetamol;although, he has an indwelling catheter for incontinence. Following days, he had an arterial leg ulcer on right angle for which daily dressing were done. Presently, he is afibrle and was able to walk with pick up frame;however, he requires assistance with activities of daily living and dressing.
Mr watson lives alone and he has a supportive daughter, who visits frequently. He is allergic to Amoxicillin and penicillin. He receive help from Ivanno day centre and Heidelberg council home support. He is worried about managing at home after discharge.
As per discharge plan, it wold be greatly appreciated if you could monitor him and assist him with showering and dressing. Kindly change the urinary catherer every 6 weekly.
