Re:Mr Adam White, DOB: 25 July 1958
Dear Nurse,
Iam writing with regards to Mr White, who has been suffering from bowel cancer and GERD, for which he requires urgent colonoscopy and endoscopy at your centre.
Mr White presented to GP today with complains of changes in bowel function followed by diarrhea and constipation as well as blood in urine. However, he experience flareup immediately after having meals and his bowel cancer test revealed inconclusive results. Therefore, he has been waiting for these procedures since 3 months. Mr White is a social smoker and a heavy drinker.
He lives alone and have irregular eating habits. In addition, he has a history of appendectomy (1988) and GERD(2017).
Regarding his pre-procedure management, vitals are stable, investigations has sent which will be revealed in 3 days and no issues regarding previous anaesthesia . In addition, he has been provided a booklet which includes pre-procedure assessment and management. Simultaneously, he has been adviced for healthy lifestyle, cessation of smoking and reduce in alcohol consumption.
Based upon the aforementioned above, it would be greatly appreciated if you could encourage him for review back to the GP after procedure within one week in order to determine the course of treatment.
