Re: Mr Adam White,DOB: 25 July 1958

I am referring Mr White, who has been suffering with recent flare up of gastroesophageal reflux disease. He requires immediate colonoscopy and endoscopy for further management.

Mr White,presented with the compliants of above mentioned symptom particularly after consuming meals since 3 months.In addition, has been facing fluctuating bowel pattern such as diarrhoea and constipation along with hematochezia since 2 weeks;however, his vital signs were stable.His blood results of FBE,U and Es,LFT has been due in 3 days. Health education has been provided regarding cessation of smoking and minimise the consumption of alcohol. Apart from this, the essential of healthy diet, exercise program and Weight Loss were discussed.

A 50 year old Mr White,prefers to follow irregular and and unhealthy eating habits.He has the habit of social smoking and drinking alcohol 50 units per week.He takes i paracetamol and ibuprofen for his left leg pain when its record as well as srinethicone for flatulence and macrogol to relieve constipation. Besides, he has the allergy history of codeine.

It would be greatly appreciated, if you could do the arrangements for the earliest procedure. please note that, he has been scheduled for the review with doctor and nurse after one week following the procedure.
