Dear Dietitian,

Re: Mr Barry O'Reith, DOB: 2 July 1963

I am referring Mr Barry O'Reith, who has undergone coronary artery bypass graft following the diagnosis of obstructive coronary artery disease. He requires dietary guidance as he is scheduled for the discharge today.

Postoperatively, he attained excellent recovery after the treatment process;however, health education has been provided regarding cessation of smoking, minimise the consumption of alcohol as well as intake of low fat diet.

Mr O'Reith, has the habit of smoking 20 cigarettes daily for past 30 years and consumption of alcohol 2 units daily. He prefers to take high fat and high calorie food thus, his BMI reveals 33.Apart from this, he has the allergy history of nuts.

It is highly appreciable if you recommend appropriate diet regimen according to his medical condition and BMI. In addition provide provide suggestion to quit smoking and minimise the alcohol intake.

Pertaining to the above circumstances ,if you have any queries do not hesitate to contact me.

Yours faithfully
