RE: Mrs Kedy Fernandez, DOB: 2/04/1975


Ms Fernandez, who was admitted into our hospital in unconscious condition and has been diagnosed with diabetes ketoacidosis, requires home visit for continuing treatment and insulin management.


During hospitalisation, Blood investigations were conducted revealed severe metabolic acidosis ;therefor, IV fluids, injection norepinephrine, potassium replacement and insulin were administered. The blood glucose, electrolytes, hydration status were regularly monitored and she responded well to the treatment; thus, her potassium value levels reached to the normal limits.


Her husband reported about she is being non-compliant with medication and diet specially with Insulin, the reason she fainted before admission.


For avoiding emergency situations in future, education and importance of having insulin on time have been provided included having balance diet on time.


Today she is going to be discharged it would be appreciated if you could visit her and provide her diabetic education in order to minimise future emergences. Kindly show her the technique to monitor urine ketones. Encouragement to maintain metabolic control and importance of having insulin are recommended.
