Re: John Alex Frederick, DOB: 18 June 1987

Mr Frederick, who is under our rehabilitative care by home visits followed after a fall down injury, requires your daily visits.

Mr Frederick had a episode of stroke three years ago, as a result he underwent the emergency clot removal surgery. Since than his mobility has impaired and speech was sullied. He had a fall down injury at home therefore he he has been receiving assistance for daily dressing and showering through home visits.

As per assessment, Mr Frederick’s knee was well healed and has no signs of infection. Although he can mobilize with the help, experiences usual pain. He’s current medication is tab. amoxil, 500mg,ones daily, for three days. Today, he can able to mobilize with walking sticks. His wife has requested for home visits of physiotherapist in order to improve mobility.

I, therefore, would like to request you to prove assistance to Mr Frederick by your regular visits.
