Re: Mrs Margaret Goldenstein, aged 61.
I am writing to return Mrs Margaret Goldenstein back to your facility who is recuperating from a dislocated right elbow. She requires ongoing care and further assistance after her discharge today.
Mrs Goldenstein, was admitted to the hospital on 12 November, 2016 as above mentioned diagnosis and early dementia progressing rapidly. She was treated appropriately, right elbow was supported with a sling. Although, healing process has been slow but skin is intact. She was prescribed a tab. Atacand plus 32/12.5 25mg daily, tab.Remeron 15mg per day,tab. Oxycodone 5mg three times a day. In order to control the pain to be continued if necessary.
Hemodynamically, she is stable.
During hospitalization, she has had bladder and bowel incontinence. Please note, she is reluctant to wear disposable briefs. Apart from that, she is ready for discharge. It is important to know that her husband will accompany her to transport back to Eventide.
As per the above circumstances, she needs to be followed up with a physiotherapist. It would be greatly appreciated if you could monitor healing of the elbow and assist in activities of daily living. Moreover, observe incontinence care. She has been scheduled for an appointment on 28 November, 2016 at 2pm at Eventide.
