Dear Dr Farquad,
Re: Ms Tabitha Taborlin, aged 45
Iam referring Ms Taborlin, who has had type 1
diabetes mellitus since childhood, requires
further assessment and management following
her discharge today.
Ms Tabolin was admitted on 02 April 2018 with
Diabetic ketoacidosis, for which intraveneous
fluids and insulin were administered.On
examination, 530mmol/L blood sugar was noted
for her.During hospitilization, she was educated
regarding the importance of glucose testing,
insulin administration and smoking cessation;
however,she has been maintainig non-compliance
and insulin regimen.In addition,discussed
regarding microvascular and macrovascular
complication of diabetes.
Ms Tabolin lives alone and works as a teacher.
She has a history of multiple episodes of diabetic
ketoacidosis and she takes lanctus 45 units at
night and preprandial humalog with 12 units.she
is obese for her age, her body mass index is 18.2
and has habit of smoking.
In view of the above, kindly provide pamphlets
and refills of insulin injection for Ms Tabolin.
It is imperative to provide a strict glycemic
control and possible insulin pump.Please note,an
appointment has been scheduled after one month.
