Dear Nurse-In-Charge,
RE: Ms Margaret Helen Martin, Aged: 82years old.
I am writing regarding Ms Martin, who was admitted to our hospital on the15/07/2018, for coronary artery disease and angina. Due to making excellent progress, she is scheduled to be discharged tomorrow (16/07/2018) and now requires your continued monitoring and support.
During hospitalization, she was treated with angioplasty. she is wearing a hearing aid. She was then prescribed aspirin; clopidogrel; metoprolol; ramipril and atorvastatin. she was able to tolerate a light diet and fluids, and there were bruises at the catheter site during insertions but there were no signs of infection or bleeding.
It is worth mentioning that she has a family history of coronary heart disease.
It would be greatly appreciated if you could arrange a daily home visit from the community nurse, dietician, and physiotherapist for monitoring of adherence to post-operative medication, exercise, and dietary regime. The dietitian should ensure a low-calorie, high=protein, low-cholesterol, gluten-free diet, frequent small meals with plenty of water. She should do light exercise without heavy lifting for 12 weeks. Please kindly monitor her wound site for infections and bruises, and arrange regular family visits to monitor her progress.
