Dear Ms Davone,


Iam writing to refer Mr Peter Hendrick, a truck driver has been arrived to hospital with chest tightness and later diagnosed as myocardial infraction. He requires advice and encouragement from your care after being discharge today.


Mr Hendrick brought to hospital on 2 July with IV access in ambulance and with the complaints of discomfort in jaw and chest as well as heaviness in both the arms. However, he has been diagnosed as myocardial infraction, for which he underwent emergency angioplasty, and then he has been proceeded with stenting to proximal LAD because of the ST elevations shown in ECG. Moreover, Mr Hendrick's lab revealed high levels of cholesterol.


Mr Hendrick is a truck driver and measuring a weight of 85 kg with BMI 29. He has the family history of heart diseases. Therefore, he has been educated regarding weight loss and cholesterol .


Upon discharge, Mr Hendrick's cardiac status is well maintained and he has scheduled for physiotherapy for cardiac exercises and dietician review for weight loss and cholesterol. So it would be greatly appreciated if you could advice him not to drive for 6 weeks and encourage him to normal daily activities.
