Dear Dr Halford,

Re: Holly Ann,aged 6

I am writing to refer Holly who has been diagnosed with hypoxic brain injury due to near drowning. She is being discharged today and requires follow up care and management.

Holly was admitted to this hospital with severe life threatening symptoms like chaotic, pulseless, apnea along with fixed and dialated pupils. Her tympanic temparature was 26.7degree centigrade. Therefore paediatric ventilator was used and spontaneous circulation was improved.

On Subsequent days Holly's condition improved and was transferred from intensive care unit to general ward. However slurred speech has been eminent, for which speech therapy was initiated. Also, her parents concerned about prominent speech impairment . Even though neurological examination was done which was unremarkable. In addition, neuro psychological, visual sptical and intelligent quotient are recommended.

It would be greatly appreciated if you could provide care for Ann. Please advice the parents about the importance of paracetamol. Kindly monitor her cough if it worsen return to hospital immediately.

Thank you for accepting the care of Holly . If you have any queries, please do not hesitate to contact me.

Yours Sincerely,

Charge Nurse
