Re: Mr Joan Cummings,aged 81 years
I am writing to refer Mr cummings, who has been diagnosed with anxiety syndrome and postural hypertension following the falls. He is being scheduled for the discharge today.
On examination,multiple bruising were noted.Subsequently, in 2000 underwent a mastectomy following a breast cancer. However;he has dementia since 2008 and 2010 had episode of vertigo and left neck of femur fracture due to a fall.
During hospitalization, computed tomography was performed to determine postural hypertension. Ice packs applications provided to the bruised area when it is required. In addition to this,an exercise program with Physiotherapy conducted with the help of his hip protectors and walker to improve ambulation and daily living activities. Besides that diet and fluids monitored. Medication compliance such as paracetamol 1gm and oxycodone in 5 -10 mg four times as required, caltrate plus two tablets in the morning,aicept 10mg, coloxyl and senna 1-2 tablets at bedtime.
To enhance optimal level of independence with daily living activities and mobility physiotherapy to be continued. Please note that, he is scheduled for follow up with physician and optimal optometrist.
He lives with her daughter whereas family is reluctant to care of him.He preferred to spend time in the garden.
Pertaining to the above circumstances, monitor medication,diet and assistance in physiotherapy is required.
