Re: Mr. Martin Jones, aged 69
I am writing to update Mr Maxtin Jones, who requires your home visits for the prescription of Syringe driver and medicines for pain relief.
On 8 August, Mr. Jones has been under home palliative care and nurse visited on daily basis. On first home visit Mr Jones was Some days, his weight was reduced 2 kg His daughter was concerned regarding his non-compliance medication and diet.
On 31 August, he was experience difficulty in swallowing and food intake; therefore he was tolerating fluids and medications On next visit he was not taking food and medications for which syringe driver was offered to them but they reluctants. Pain was reported for which Liquid marphine and topical cream were administed..
Today's he was complaint of severe pain and he has not take his medicine and food. Now, they agreed to tape. syringe driver.
Based on the aforementioned circumstances, you could provide, education regarding on. syringe driver and adminstration of medicines it would be beneficial for Mr Jones to his speedy recovery.
