Dear Mr.Ortner,
Re:Mr.Nicl Daniel, aged 68.
This letter to introduce Mr.Nicl Daniel who requires physiotherapy and advise regarding exercise following his discharge. He was admitted to our facility and underwent hip replacement surgery following an osteoarthritis.
On his first visit ,he presented with pain and decreased internal rotation along with flexion on the right hip. On subsequent days, he reduced range of motion and unable to weight bear. In addition to this his condition was not responded to anti-inflammatory medication there fore surgery was performed on 19 July 2019.During hospitalization physiotherapy was performed for him.Presently,he is on analgesics and calcium supplements.
He has been history of diabetes type 2,lactose intolerance and allergic to penicillin. He is a retired farmer lives with his wife in story house farm.
In view of the above ,kindly request to teach him about walker,crutches,weight bearing exercises and climb on stairs after full recovery. Your special concern is highly appreciated.
