RE: Ms Kylie weiss, DOB: 21 May 1952

The request is being written regarding the above-mentioned patient, who has been recovering from myocardial infarction and ready to be discharged today, requires your assistance to enquire about eligibility of medical allowance and other benefits.

The patient has a daughter and lives with her husband who is retired and a pensioner. She works as a taxi driver but would not be able to work for six weeks due to sickness as a result, a concern has been expressed about being unable to manage home with only husband’s pension: therefore your inputs are required if you contact the Australian Government Department of Human Services.

Your inputs will be beneficial to provide her assistance whether she is eligible for the sickness allowance and if any other benefit will be provided would be much appreciated.
