Re: Mr Gerald Baker, aged 79
Mr Baker who has been recovering from left THR requires requires a continuing care, appropriate management and monitoring medications under your guidance. He had a history of Osteoarthrits from 2016 and also known Hypertensive for which he is on rampiril 5mg, Panedeine forte 2 tablets , 4 tmes a day and asprinin 100mg taking post-operatively.
During hospital stay, the surgical site dressing done once in a day for early healing, encouraged to perform good range of motion, stretching qnd strengthing exercises and occupational therapy. Excellent mobilization have been noticed post-operatively which included wheelie walker for weight bearing and walking with ease. Eventually, during his recovery disorientation was exhibited which is likely related to sedation drugs. It was also evident that post-operative Hb level decreased by 72g/dl for which 3 units PRBC’s were transfused and suggestive to monitor for the levels.
On discharge to have check on medications especially, Pandeine forte as it may cause drowsiness, maintain good skin care, encourage daily exercises and removal of staples surgical site with in 2 weeks. FBC and UEC investigations check up to be done in City Hospital clinic. In addition, walker and wedge pillow provided by hospital and social worker arranged for toilet raiser by local medical supplier for 2 weeks.
