Dear Nursing Director,
Re: Ms Sydney Loten, DOB: August 14, 1946
Ms Sydney Loten who has been recuperating from colon polypectomy, requires ongoing care and assistance following the discharge today.
On April 11,2020, Ms Loten underwent routine colonosiopy , which revealed multiple polyps for whicg he underwent surgery, post-operatively, combinations of treatments are used to manage her condition appropriately. Walking is possible with the assistance if the cane where as oxygen one litre is required to maintain the saturation 98%. Periodic bowel movements and slight pain at incision is noticed. Adult diapers are required for incontinence.
Ms Loten lives with son; however , she is often . “Unable to look agter due to his busy scheduled work. She smokes 5 cigarattes per day and consumes one bottle of beer per week now. She has had history of diabetes and hypertension since 2007, 2008, for which she is on tab metformin 500mg twice daily and glopixide 5mg, losartan 40mg once daily, respectively. She also has COPD for which ipratropium bromine inhaler 1 puff was prescribed, where as budosonide plus formated of 2 puffs twice daily and prednisone 40mg once daily was prescribed for acute attacks.
Continuation of regular medication compliance and requirement of transition care programme was explained. Regular dressing change at home has been recommended. Continuous monitoring of O2 consumption has been advised and a follow up check up on April 30,2020 has been scheduled.
Based on the aforementioned circumstances, providing ongoing care is extremely beneficial to improve Mr Loten’s condition. In case , any additional information is required, please do not hesitate to contact me.
