Dear Nurse
Re: Mr Alfred Billy, aged 53
Mr Alfred Billy who underwent an operation is getting discharged today and required follow-up at his residence. He had been diagnosed with Basal Cell Caccinoma at the neck therfore a skin biopsy was done suggestive of a nodular carcinoma and procedure was done to remove CCB. Post- operatively, mild infection noticed at the wound site, which involved the sutures breakdown which was a suggestive of incomplete excision.
The patient social history reveals that has is a chronic smoker and alcoholic for paust 30 years.
During hospitalization, the healing appeared normal with no evidence of swelling. Panedein Forte, 500mg was administered to control pain. The patient has not exhibited any complications followed by surgery and was coping well without difficulty.
On discharge to check and analyse the wound daily for good healing and to continue the Panedein Forte 500 mg for 7 days.
If you require further information, please do not hesitate to contact me.
Yours faithfully,
Charge Nurse
