Re: Ms Kylie Weiss, DOB: 21 May 1952


I am writing to request a home visit for Ms Weiss, who requires an assessment on the eligibility for sickness allowance and benefits from Australian Government Department of Human Services.


She lives with her husband, Mr Peter Weiss, who is a 67-year-old retired pensioner and has a daughter.She is a taxi driver and has mixed shifts due to which she consumes fast food, as a result overweight is noticed with body mass index 29.


On 7 July, she was brought to hospital with 2 hours history of intermittent discomfort jaw and heaviness in both forearms, for which she has undergone an emergency angioplasty and it was uneventful. Now, her cardiac condition is stable without further complications. During hospitalization, education has been provided regarding her medication compliance and lifestyle modification.


Upon discharge, cardiac rehabilitation has been recommended for weight loss and low-cholesterol diet. In addition, she was advised to avoid driving for next 6 weeks. At the time of discharge, she was worried about servive with only her husband's pension. Therefore, social worker assistance is referred to resolve her financial issues.


Based on the aforementioned circumstances, evaluation on the sickness allowance and benefits from human services would be extremely beneficial.
