Re: Mr Jamie Morgan, DOB: 22February 1956


Mr Jamie Morgan, who has been undergone a coronary artery bipass grafts, now requires instructions on low fat dietary and healthy simple recipes. Your further assessment would be highly appreciated.


Regarding the patient, social and medical history; Mr Morgan who have not been married and lives alone in his own home. He has been a heavy smoker with 30 cigarettes and alcohol 3 multiply 300ml bottles per day. Furthermore, he has been an allergic to peanuts. His height 177cm and weight 96kg as noted. Moreover, his diet includes sausages, deep fried chips, burgers, eggs and MacDonalds.


Mr Morgan was advised a routine post operative recovery. Hence, he was advised regarding the importance of cessation of smoking and to decrease the alcohol intake and commenced on low fat diet. His routine visits with social worker.


As per the discharge plan, he is been returning back to Goondiwindi. His appointment has been arranged with local Gp Dr.Stevenson George on 10 October 2017 at 2pm and rehabilitation exercise to continue with local physiotherapist.
