Dear Mr Simon
Re: Mr Jamie Morgan,D.O.B: 22 February 1956
I am writing on behalf of Mr Morgan who is recuperating from coronary artery bypass surgery, requires suggestions and ideas for healthy simple recipes as well as guidelines on low cholesterol diet.
On 27 September 2017, he was admitted with obstructive coronary artery disease for which coronary artery bypass surgery was done four days later . Postoperative recovery was usual with gradual wound healing and good mobilization was noticed. Discharge education was given to avoid smoking and alcohol, and to follow healthy, low cholesterol diet.
Mr.Morgan resides in his own home, works as fencing contractor and unmarried. He is chronic smoker and alcoholic usually smokes 30 cigarettes/per day and drinks three bottles/ day respectively. Dietary habits includes fast foods and unhealthy diet as a result the BMI is above normal, classified as grade 1 obesity.
Based on the above mentioned details, appropriate dietary regimen and changes in life style habits should be followed by your guidance and advice. I request you to send the information details to his home address where I am attaching the address to this letter.
If you have any queries, please do not hesitate to contact me.
Yours Sincerely
