Re: Mrs Helen Marshal, D.O.B: 29/01/1968


I am referring Mrs Marshall who requires emergency admission as suspected clinical symptoms of Acute Myocardial infarction like sweating, severe and radiating pain, chest tightness were noticed, therefore oxygen 2 lit/min was administered and Nitroderm patch was applied. Pain was unreleaved with buscopan and panadol so immediately patient was transferred by an ambulance.


Mrs Marshall has been diagnosed with hypertension and Glaucoma since 10years for which captopril 25mg twice daily and eye drops were used respectively along with some over the counter medications for pain and indigestion. A walking frame is being used for mobilization.


Regular complaints of epigastric pain and indigestion were noticed, as such buscopan and panadol 2tab used to relieve symptoms. But today since 19:45 symptoms of tiredness,weakness,anxiety,trouble sleeping,increased blood pressure of 180/95 were observed and informed GP about the situation. Due to his schedule he admitted to visit the next day.


In spite of administering medicines for alleviating pain and other symptoms, her condition worsened more of anxiousness,depression and rated pain score as 8/10 . Immediately necessary precautions were taken suspecting as Myocardial Infarction.


Based on the above mentioned details, I request to admit and provide appropriate management .
