Re: Ms Akira Annaisha, aged 5


I am writing to refer Ms Akira Annaisha, who was diagnosed with acute meningoencephalitis, requires monitoring of complaince with vaccination schedule and complete follow-up for the family.


Ms Annaisha was presented with acute meningoencepthalitis following mumps. Complete recovery from mumps and acute meningoencephalitis were reported. A monthly neurological assessment is mandatory for next three months. Additionally adviced is gived on vaccines for both children Akira and his sister Emica.


Akira lives on rental house with his prenta and one subling. His father works as a shift worker in a boots factory and mother is a housewife. Family have been recently migrated from Japan and they do not understand the communication skills. Family has not contact with general practitioner due to delay in processing of migration.


Eventhough both children had been vaccinated at birth their record was last lack of awarness regarding MMR vaccination was noticed in the family.


Based on the aforementioned circumstances, your regular monitoring for vaccination and follow-up care for family would be highly beneficial.
