Dear Dr. Johnson,
Re: Mr. John Williams, aged 68 years
Mr. Williams who has been diagnosed with angina after underwent a myocardial perfusion scintigraphy. Your care, continuous monitoring and further management are recommended.
On 10 April 2018, be presented with the complaints of pain, aches, discomfort and tightness across the front of the ‘chest’. On observation his blood pressure showed 170/110mm Hg.
Mr. Williams sensory vision is normal with spectacles: however, have mild hearing issue. His speech is clear with mild dysphasia. Although he ambulates independently with the help of cane or rolling walker, he needs supervision on the stairs. He has a incontinent of bladder for which he uses disposable undergarments. Please note that , he has a habit of smoking.
It would be appreciated to continue his care and evaluate for further treatment.
