Re: Mr Amir akbari, aged 41


Mr Amir akbari,who was diagnosed with gullain-barre Syndrome,is being transferred into your care today following discharge.


Mr Akbari was admitted on 06 March 2011 with inability to move 19mbs can be related to decreased muscle tone and strength. During hospitalization rapid detoriation of health was noted. Therefore, ventilatory support was provided in the intensive care unit for 3 days. Although, initiated regular plasmapharesis intravenous gamaglobulin for the period of 5 days. Mobilization is possible with the support of a walking Frome, certain amount of assistance is definitely required for performing activities of daily living.


Mr Akbari has a history of post-traumatic stress disorder because of war trauma as of 80's, depression and anxiety has developed amid hospital stay.


Mr Akbari lives with family at rented house,belongs to muslim and Iranian, preferred to have halal food.


Based on the above mentioned circumstances, participation of physiotherapy programme,and self-exercises highly recommended. An arrangement for physiatric assessment should be beneficial to stabilize the depression. Make sure for councelling to his wife stress with the involvement of the social worker. Regular monitoring of the respiratory status and for the neurological signs would be greatly appreciated.
