Re: Mr peter Dunbar , DOB : 18 march 1933

Mr peter Dunbar is being transferred to your facility today and requires monitoring of his diabetes and encouragement to comply with his medications and diatery region

Mr Dunbar has had diabetes since 2018 for which tablet metformin 508 mg thrice a day was commanced however he remains non complaint to medications and dietary regions , consumes excessive amount of alcohol , fatty foods , salts and sugars contrary to professional advice and blames poor memory as the fundamental cause of his neglect .

In june 2018,he suffered a myocardial information and was hospitalised at the city hospital in newtown . on the same admission , atrial fibrillation was diagnosed and subsequently , sotalol and warfarin were commanead. Hypertension is effectively controlled by ramipril .

Presently he mobilises with a walking stick as a concequance of diabatic neurapathy and has poor exercise tolerance .

Based on the above , circumstances regular supervision and encouragement would be extremely beneficial to improve Mr Dunbar's condition in case you require any additional information please don't hesitate to contact me .
