This letter will accomany ms jane simms, who was diagnosed with progressive for scterosis, requires regular homevisits for ongoing care and management she is being discharged back home tomorrow.

as you are aware , on 26 of july 2018, ms jane was admitted for the above mentioned reason. during hospitalization , her treatment commenced on prothiaden 150mg daily along with her regular medications. daily dressing was applied on the right buttek and low calorie diet was initiated she has to undergo a range of motion , stretching , strengthening exercise and occupational therapy . over all, she has made excellent progress.

based on the above mentioned circumference , regular monitoring of medication compliance is extremely recommenced; monitoring weight is required due to recent exacerbation of obesity and keep an eye pressure area to pressure skin integrity . it is also necessary to continue exercise program and encouraged to participates ant new or interests activities inorder to reduce her depression.
