Re: Mrs. Anne Ellis, aged 76
Mrs. Ellis, who has bilateral multiple renal calculi, and hydronephrosus requires continuing care and management following the discharge today.
On 12 June 2018, Mrs. Ellis was presented with severe pain, score 8/10 at the universities, left joint and groin region , which radiates to the lower back. Vomiting, sweating, dehydration, restless, grimace and haematuria were noticed. Subsequently, intravenous Diclofenac 75mg for pain, Metocopramide 10 mg for vomiting, and intravenous rehydration were administered computerized Tomography scan was performed, showed showed dilation of left collecting system and obstructed pelvicureteric junction calculus.
On the following, Tamsolusin, 8mg twice daily and paracetamol 500mg every six hour were commenced , gradually pain reduction has been reported. Urology evaluation performed which revealed, ston will pass spontaneously fortnight. Strict complaince of oral fluid intake of two to three litres daily recommended. Dietry restriction regarding, salt, oxalated rich foods, and animals protein were advised.
Based on the aformentioned circumstances; regular monitoring of urine output and checking of passage of stone and to be sent to laboratory for analysis is profoundly recommended.
