Re:Mr Gerald Baker,aged 79


I am writing to inform you regarding Mr Baker.He is being discharged from city hospital back into your care.He underwent left total hip replacement surgery

Postoperatively, he was commenced on Aspirin,100mg.In addition to that Ramipril,5mg nad panadeine forte 2 four times a day.He requires daily dressing over the surgical site.He is maintaining good mobility after the surgery with the help of wheelie walker.Hence he requires range of motion,stretching and strengthening exercise along with that occupational therapy is also needed.

Mr Baker appears disoriented during post operative that May relate to anaesthesia.He transfused 3 units of packed red blood cells as his hemoglobin was low post operatively. How ever hospital will provide walker and pillow and apart from that hospital social worker organised 2 weeks hire of raises from local medical supplies.

It would be greatly appreciated if you could provide continuing care and management. Please note,Mr Baker will have his staples removed at city hospital in 2 weeks and also he have followup fasting blood count and urea,electrolyte,creatinine test also.
