Re: Mr Walter Pitman, aged 69
I am referring Mr Walter Pitman, back into your care for further management and cardiac assessment in regard to elevation of blood pressure.
On 01 July2018, Mr Walter Pitman was initially admitted with deep laceration on lower left arm, on admission, wound was cleaned, sutured and dressed . Commenced on intravenous antibiotic course till 2july 2018, which were changed to oral antibiotics. During third day of hospitalization, regular visits to outpatient clinic for changing dressing is recommended.
On subsequent visit, Mr Pitman presented with complaint of excrutiating pain along with mild hematoma. Further assessment revealed raised blood pressure190\86 mm hg in supine position and 186\89mmhg in sitting position. However, wound has recuperated well, whereas blood pressure readings are still at peak from the past 7 days. Currently, blood pressure is 190/84mmhg while sitting and 196/86mmhg in lying position.
On the basis of a formentioned circumstances, it would be greatly appreciated if you provide complete assessment in order to find the exact cause for mushrooming up of the blood pressure. Adequate treatment on the basis of assessment would be highly beneficial for improving Mr Pitman’s condition.
