Dear Ms Howard,
Re: Ms Bestie Anderson, aged 89
Iam referring Ms Bestie Anderson, who was diagnosed with urinary tract infection into your care for further assistance and appropriate management following discharge today.
On 16 Feburary 2020, Ms Anderson presented with complaints of urinary incontinence along with, low grade fever. Therefore, commenced on acetaminophen 650 mg orally and injection ciprofloxacin intravenously. Drowsiness and sleepiness was evident on admission. On further investigation elevation white blood cells as well as presence of bacteria in urine was noted.
Ms Bestie Anderson has had history of hypothyroid, dementia, constipation, gastric reflux diseases and undergone a right hip surgery in 2017. Tablet Levothyroxine 200mcg, protonix 75 mg daily colace 100 mg morning, ciprofloxacin 250 mg orally after every 12 hours for fourteen days and Acript 10 mg in night was prescribed for managing condition at home.
Subsequently, Ms Anderson’s condition has improved with reduction in solomence and disorientation. Assistance provided in accomplishing activities of daily livings and physio theraphy. Appropriate hygiene with frequent toileting is ensured. Regular assessment of vital signs and loss of control is advised.
On the basis of afore mentioned circumstances, it would be greatly appreciated if you could schedule follow up visit with Dr Alex Rose in 2 weeks with repeating urine culture. Beside commode, cane and walker are recommended for aiding in ambulation. Regular surveillance for the signs of dehydration and orthostatic blood pressure is imperative to improve Mr Anderson’s condition.
