Re: Alicia Nesbitt, age 6 days old

Dear Midwife

Alicia Nesbitt, who is recovering from low birth weight and opioid dependence, requires ongoing care and management. The baby and mother being discharged today.

On 22 April, Alicia was delivered with a weight of 2.0 kilograms, which is currently increased to 2.3 kilograms. During hospitalization, loss of appetite was observed that possibly due to improper bottle feeding. Apart from this, poor bonding between mother and baby was developed. Eventhough, she has less affection to her child, she often needs prompted care. Currently, mother and baby have been completed heroin withdrawal treatment without complications.

Alicia’s mother, Ms Pool is a 20 years old single mother and is a sex worker. She was an heroin dependent for the last two years. Besides that, she estranged from Alicia’s father during her pregnancy period. Hence, Alicia has been advised to stay with grandmother after discharged.

In view of the above, regular visit for the monitoring of feeding techniques and weight in Alicia’s condition is extremely beneficial. Evaluation of coping and psychological state for Pool’s condition is recommended. Education on infant care for mother and grand mother is suggested. Kindly arrange a drug and alcohol team to encourage her to quit heroin use.

The doctor’s notes will be forwarded to you. In case you required any additional information, please do not hesitate to contact me.

Yours faithfully
