Dear Mrs Penny,

Re: Ms Sheila Cartwright, aged 69

Iam writing to update you regarding Ms Sheila Cartwright, who is being discharged today. She was admitted with unstable diabetes mellitus and infected left foot ulcer.

Initially, Ms Sheila Cartwright has had non insulin dependent diabetes for the previous 15 years. Additionally, periodic problems were reported regarding self- administration of hypoglycemia medication. Therefore, continuation of medical hypoglycemic agent (glibenclamide) is suggested. Regular daily saline dressing are required for the left foot ulcer. However, wound margins and signs of complication were observed and healing progress is reviewed. The current medication of Ms Cartwright are Metformin850mg thrice a day , Glimel 5mg daily and Augmentin duo forte 125mg orally twice a day . A review is required for antibiotic theraphy at the completion of the ongoing course

Ms Sheila retired in 2007 at 65 from managerical position, alcohol consumption has darastically increased, whereas the dietary quality detoriated tremendously. She resides in a personally owned four-bedroom home and is dependent on meager position. She leads a lonely and sedentary lifestyle. She lacks any personal interest and hobbies.

As per the above-mentioned circumstances, it is recommended to gradually minimize alcohol consumption. Regularly monitoring is highly necessary for ensuring medical compliance. Observing Ms Sheila’s blood sugar level is recommended. Improving dietary quality by providing nutritious components is suggested. Encouragement for moderate exercise programme as well as establishment of social activities and income producing activities is suggested.
