Re: Betsie Anderson, aged 89
Ms Betsie Anderson, who was diagnosed with urinary tract infection, is being discharged back into your facility today. She requires on going constant monitoring and appropriate assistance.
On 16 February 2020, Ms Anderson was admitted in emergency department in Norfolk and Norwich University Hospital. Vital signs and laboratory investigations were approached, which revealed tachycardia as 119 beats/ min and low oxygen saturation on room air. White blood cell count was reported as 15.2mg/dl as well as urinalysis for bacterial infection was evaluated. Tablet Acetaminophen 650mg was commenced orally once. Also, intravenous Ciprofloxacin has been initiated.
In addition, regular monitoring of loss of consciousness and blood pressure was performed. Assistance with activities of daily living activities and frequent toileting with proper hygiene was provided. Ms Anderson is well oriented to time, place and person and started performing activities under a minor guidance.
Therefore, continuous daily evaluation of vital signs and general assistance is highly required. Observation for signs of dehydration and orthostatic blood pressure is advised. Review for urinalysis and blood investigations along with follow-up with the general practitioner Dr Alex Rose are scheduled after a fortnight. A Walker for ambulation, a cane and bedside commode are required for incontinence of urine and bowel. The current medications list is attached with this letter for your kind perusal.
