Re: Mr Tom Clarke, DOB: 21 September 1954


I am writing to request home visits for Mr Tom Clarke, who is recovering from fracture of coronoid process ulna and radial head fracture in left elbow. He requires supportive care and rehabilitative program.


On 26 July, Mr Clarke presented to emergency department with injury in the left elbow following to an accident. X-ray confirmed his diagnosis. He was advised for sling for 6 weeks and commenced with pain killers. On subsequent visit, X-ray reveals injury heals well. Sling were removed, exercise program was initiated.

On today's visit, Mr Clarke has complaints of of limited range of motion and stiffness in the elbow. He was recommended with rehabitation program which will be supervised by physiotherapist.


Mr Clarke is a retired post officer, lives with his wife. He has two daughters. He had a history of right ankle dislocation surgery ,septoplasty and hypertension which was managed with ramipril.

In view of the above circumstances, Your expert guidance would be e beneficial to Mr Clarke's to restore strength and mobility.
