Re:Ms Margaret Helen Martin

DOB:25 July 1935


I am writing to you regarding Ms Margaret Martin who require anticipated care and monitoring from your service. She has been diagnosed with coronary artery disease and angina.She underwent repeat angioplasty.She will be discharged tomorrow.

Ms Martin is an independent women who lives in own house in Greenville.She is maintaining an intermittent contact with her family.Her medical history reveals that she has have coeliac disease, hypertension, hypercholesterolemia. Hence she has have commenced with aspirin,clopidrogrel,metropolol,ramipril and atorvastatin

Ms Martin is overweight (BMI - 29.5)and sedentary. She is anxious about returning home.She wears hearing aid for hearing loss. Post operatively, she is tolerating to light diet and fluids.Bruising was present over catheter site with no signs of infection and bleeding.

Ms Martin needs low calorie,high protein, low cholesterol,gluten free diet with frequent small meals including snacks and fluids.She requires daily light exercise and it would be monitored by physiotherapist. She has to avoid heavy lifting for twelve weeks.

It would be greatly appreciated if you could provide a nurse for home visit to monitor her adherence in post operative medication, exercise as well as in diet regime.Please note that, she requires regular monitoring by doctor,dietician and physiotherapist.
