Dear Director,

Re: Master Vamuya Obeki ,DOB:23 May, 2018

I am writing to refer Master Vamuya, who is fully recovered from mumps and acute meningoencephalitis , as his parents requires follow up advice regarding vaccine from your service .Vamuya is being discharged today.

Master Vamuya's parents Mr Abdullah and Mrs Miri who were refugee ,arrived in Australia in 2012 .His father is a shift worker in Golden Circle Pineapple factory and his mother is a housewife. He has a brother Saeed ,aged two. They lives in a rental accommodation .Vamuya's parents understand Limited English hence interpreter is needed .

Vamuya's parents were unaware of vaccines for mumps and stated that both children had received vaccination at birth but record is not available .

In view of the above details ,it would be greatly appreciated if you could advise Vamuya's parents on recommended vaccine for both children . Please note that he requires neurological check up.

Please do not hesitate to contact me for any further queries.

Yours faithfully ,

Nurse

Children's Emergency Department
