Re: Miss Allison Cooper ,DOB :14 June ,2002


I am writing to refer Alison grade 5 student, who requires an assessment and further treatment from your service. she is not activate at class as well as home followed by her father's death .


Since grade 3, Alison had trouble in concentrating in school and avoid participating in Sporting activities. Her weight is above the normal .She has Eczema on hands and mild astma for which she applies oinment and inhales ventolin inhaler respectively. She took regular absences with complaints of headache, stomach ache, aching legs and feeling nauseous for which her grandmother takes home after school.


Alison father died in a motorbike accident ,18 months ago .Her mother is working full-time as a bank manager and her grandmother lives near school who provides care after school and at holiday. Alison's mother stated that she often talks about her father and cries as she misses him. She found relax in food such as chips and cakes .


In view of the above details, it would be greatly appreciated if you could assess her problems and treat her appropriately.
