Re: Mr Dallas Walters, aged 51
I am writing to refer Mr Walters, who underwent an insertion of Continuous Ambulatory Peritoneal Dialysis catheter for chronic renal failure ,as he requires an ongoing supportive care and further management from your service after being discharged today .
Mr Walters was admitted to the hospital on 16 June, 2011 for the above mentioned procedure ,as his condition worsened recently .He has had type 2 diabetes for which he is compliant with oral medications .During hospitalization, he was admitted for a long period as both Mr Walters and his wife learn the management of CAPD slowly because he has mild chronic delusions and his wife has mild intellectual disability. Mr Walters was seen by a mental health liaison and reported that there is acute changes in psychosis .Mr Walters lives in the demountable home in Bunbury park and is on disability supportive pension for schizophrenia .
In view of the above details, it would be greatly appreciated if you could monitor his signs of peritonitis at catheter site. Please educate him on minimising potassium from died as his result was 4.0 and on discharge 3.5 to 4.8 . Provide education on CAPD and CRP for both Mr walters and his wife. Please note that his medication regimen will be forwarded by medical officer. Could you please observe for sign of psychosis increased ,if so refer as required .
