Re: Mrs Binitta Mekhal, aged 50


I am referring Mrs Mekhal back to your service, who underwent emergency hemodialysis, requires ongoing care and management following her discharge today.


On 10 August 2019, Mrs Mekhal was admitted to the hospital with complaints of severe dyspnea, swelling all over the body, and associated ailments. After the assessment, she has undergone a urgent dialysis, and was non-compliance with medicine and diet. She has had improper diet habits including consumption of deeply fried and fatty foods, and weight loss has been noticed. Therefore, she was recommended with dietary follow-up after being discharged. Proteinuria and electrolyte imbalance were noticed, and health education was provided for the cessation of alcohol and smoking habits. Today, excellent recovery was noticed.


Pertaining to the above-mentioned situation, kindly take over the case of Mrs Mekhal and provide continuous assistance and support. Assistance in daily activities, observing medication compliance, and monitoring associated side effects, as well as avoidance of potassium restricted, and protein-rich diet would be profoundly beneficial. He has been scheduled for an OPD follow-up after a week. All the medication list is attached with this letter for your kind review.
