Re: Mr Lional Ramamurthy, Aged 63


Mr Ramamurthy is back into your care who diagnosed with Pneumonia. He requires ongoing care and assistance at your facility.


Mr Ramamurthy was admitted to hospital on 04 April,2014 with complaints of wheezing, fever,persistent cough and genalised ache.

On admission, although he was mobilising with pick up frame, he had shortness of breath on exertion for short-distance ambulation.


Currently, Mr Ramamurthy is afebrile and inflammatory markers are back to normal. He is independent to walk and perform activities of daily living but, slowly. He has weight gain after being reviewed by a dietician. However, he has a dry cough therefore, encourage him to do deep breathing and coughing exercise. He is preferred a sitting position to ensure postural drainage.


Mr Ramamurthy needs to be kept warm and administer Paracetamol in case of chest or abdomen pain. He was recommended to take increased fluids and follow good nutrition such as fruits, eggs and vegetables.
